Lessons by Nicole has put in place preventative measures to reduce the spread of Covid-19 for all in attendance, following local and state Covid-19 safety guidelines. Lessons by Nicole cannot guarantee those who enter will not be infected with Covid-19. (Zoom lessons do guarantee safety and can continue if you prefer remote lessons.)
I hereby release, covenant not to sue, discharge, and hold harmless Lessons by Nicole, its teachers, staff and owners, from the Claims, including all liabilities, actions, damages, costs or expenses of any kind arising. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of Lessons by Nicole teachers or staff, whether a COVID-19 infection occurs before, during, or after participation in any program.

- Face masks are recommended, but not required as per the NJ State Mask Mandate. I will wear a mask if requested to do so.
- Piano and high touch surfaces will be cleaned between students.
- The teacher and family will not attend if there are any symptoms in the last 48 hours, including fever, cough, chills, or extreme fatigue.
- Students will bring their own writing utensils (pencils preferred).
- Do not enter until the previous families have left. Please wait in your car until your allotted time slot.
- Hand Sanitizer will be available and should be used before entering.
- No extra time will be given for tardiness, there will be time factored into the schedule for cleaning between students.
- There will be NO bathroom usage at this time.
- Any last minute cancellations must be paid for or rescheduled, in-person students may need to go virtual for their make-up lesson

Assumption and Risk Waiver of Liability relating to Covid-19
Lessons by Nicole Liability Waiver: Assumption of the Risk and Waiver of Liability relating to Covid-19: The novel coronavirus, covid-19, has been declared a worldwide pandemic by the World Health Organization. Covid-19 is extremely contagious and is believed to be mainly spread by in-person contact. As a result, federal, provincial, and local governments and provincial and state agencies recommend social distance measures, and have, in many locations prohibited the congregations of large groups of people. Lessons by Nicole has put in place actions and preventive measures to reduce the spread of covid-19; however, Lessons by Nicole cannot guarantee those who enter the premises will not be infected with covid-19. Attending in-person lessons could increase risk of contracting covid-19 for all in attendance. By signing this agreement, I acknowledge the contagious nature of covid-19 and voluntary assume the risk that my child(ren) and I may be exposed or infected by covid-19 by attending in-person lessons at Lessons by Nicole and that such exposure or infection may result in personal injury, illness, permanent disability and death. I understand that the risk of becoming exposed to or infected by covid-19 at Lessons by Nicole may result from the actions, omissions, or negligence of myself and others, including but not limited to, Lessons by Nicole teachers, volunteers, program participants and their families. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability and death), illness, damage, loss, claim, liability or expense of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)'s attendance at Lessons by Nicole or participation in any activities associated with Lessons by Nicole. On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge and hold harmless, Lessons by Nicole, its teachers and owner, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto.
